The recent update of CHES’ disability assessment services coincides with recent changes to Maryland state laws and to the way the DDA works. They also reflect the implementation of the complex but valuable Health Risk Screening Tool (HRST).
This new tool, which is now mandated in Maryland for anyone seeking care, can seem daunting, but, as CHES explains, is actually extremely advantageous for people with a disability and their families.
Rather than assessing the level of a disability, the HRST measures a person’s health risk. The purpose of the tool is to identify health risks early and prevent future complications and identify service needs that may be less obvious. As such, if done correctly under the guidance of one of the agency’s CM or DN nurses, it can allow for greater ongoing care assistance and monitoring, and can ultimately lead to improvements in the quality of life of the person in care.
The HRST is just one of many assessments that CHES’ CM and DN nurses oversee. Through their consultation and case management services, they also assist families by conducting a comprehensive nursing assessment and a self-administration assessment, and by developing healthcare protocols, a care plan, and a connected network of healthcare providers.
The agency’s CM and DN nurses work with individuals with visual impairments, Cerebral Palsy, learning, and mental disorders, Spinal Bifida, spinal cord injuries, Down Syndrome, muscular dystrophy, Autism Spectrum Disorder (ASD), traumatic brain injuries, Multiple Sclerosis (MS) and more.
